Scheduling an Appointment

For your convenience, you may choose from one of several options in order to schedule a New Patient Consultation at Advanced Fertility Care.

OPTION 1: Complete the form below with basic contact information and one of our staff will contact you within the next 24 hours (excluding weekends and holidays) to schedule your appointment or answer any questions that you have. DO NOT send personal health information through the form below. Specific patient care questions must be addressed with your doctor during an appointment.

OPTION 2: Go directly to our secure Patient Portal Log-in Screen by clicking on the link below in order to enter basic demographic information directly into our system. You will then be called by one of our staff within the next 24 hours (excluding weekends and holidays) to schedule your appointment.

OPTION 3: Call our office directly at at (480) 874-BABY (2229) and one of our staff would be happy to assist you in making your New Patient Consultation appointment.

We check our email several times a day and will usually reply in 24 hours or less.
