Who Is Eligible For the Package Plans?
To qualify for the plan, both partners must undergo a medical screening to make sure you are good candidates for success with IVF. The specific requirements for eligibility will be reviewed with you by Dr. Zoneraich during your consultation.

* For patients with insurance coverage for Fertility, please note that Arizona Advanced Reproductive Laboratories, LLC (AARL) and Arizona Advanced Surgery Center, LLC (AASC) do not contract with insurance companies. Fees shown on our pricing pages include all fees from Advanced Fertility Care, AARL and AASC for a given procedure. Advanced Fertility Care fees will be billed to insurance if you have coverage. AARL and AASC fees will be required prior to the start of treatment.
2 and 3 cycles plans have specific contracts. Fresh and Frozen Cycles are included in these fees
